Register for the COVID-19 Vaccine Online

COVID-19 vaccines are on the way to hospitals and clinics across New Jersey. You can reserve your spot to get a vaccine now by pre-registering here. We’ll ask some questions to determine when you are eligible to receive a vaccination.

Filling out this form should take about 15 minutes, and the information you enter will be kept private with the State of New Jersey. Click below to reserve your spot.

Call the Toll-Free State Hotline for Vaccine Information

The State of New Jersey has established a new, toll-free vaccination hotline to assist residents with general questions about vaccines, identifying whether or not they’re currently eligible for a vaccination, and to help identify vaccination locations.

Vaccine Appointment Assistance for Residents 65 and Older

New Jersey residents 65 and older who have been unable to get a COVID-19 vaccine should have an easier time now that appointments are being reserved for them at designated Walmarts and Sam’s Clubs.

In Atlantic County, this includes three Walmart locations in Egg Harbor Township, Hammonton and Mays Landing as well as the Sam’s Club in Pleasantville. Eligible residents may call the stores directly to make their appointments or call the state’s 65 and older hotline at 856-249-7007, available 8 AM – 8 PM daily.

Vaccine Transportation

NJ Medicaid has announced that free transportation is available to Medicaid beneficiaries who have an appointment for the COVID vaccine. The information is as follows: 

  • ModivCare is the new name for the company that provides non-emergency medical transportation for NJ Medicaid beneficiaries to health care appointments. The previous name for the company was LogistiCare. 
  • In addition to transporting Medicaid beneficiaries to health care appointments, ModivCare also provides transportation for the COVID vaccine.  There is no mileage limitation for individuals who have a COVID vaccine appointment that is outside of their county. This is important because some people may have to travel a distance to the site of their vaccination appointment.
  • Medicaid beneficiaries who require assistance, including persons with intellectual and developmental disabilities (I/DD), are allowed to have one family member or aide with them in the car.
  • The ModivCare driver will drop off the individual at the vaccination site. The person scheduling the appointment will need to also arrange for a separate pick up time 45 minutes to an hour later.
